神秘的鼻咽部狹窄

許多貓主子在小時候一次上呼吸道感染後,一直都有很大的呼吸音,玩太瘋的時候甚至會出現張口呼吸,或是「蹺蹺板式」呼吸。貓主子沒有鼻子分泌物,症狀卻一直維持,到底發生什麽事呢?

「鼻咽部狹窄」是一個可能、也算常見的原因喔!

貓的鼻咽部狹窄一般是後天形成的,實際的「致病機轉不明」,文獻多認為是呼吸道粘膜受到刺激(如感染、創傷),導致此處粘膜進行修復、沾黏,形成「疤痕組織」,進而阻塞鼻咽部通道。

鼻咽部狹窄可以藉由鼻咽內視鏡或電腦斷層確診;治療的方式就稍微複雜了!一般治療可以歸納成氣球擴張術、狹窄處切除合併軟齶縮短的手術、暫時性支架放置、雷射疤痕組織移除術等等。一般而言,單純只用「氣球擴張術」的復發率高達(41%)(文獻1),有慢性呼吸道感染的貓復發的機率也較高。不過,若經醫師審慎評估決定合適的手術方式,也有不少病患可以在單一次手術後達到良好的效果。


那醫生,不治療會怎麼樣呢?

許多已經開始開口呼吸的貓鼻咽部開口往往只剩下針孔大小,可以明顯看到「呼吸時腹部用力、無法正常睡覺」,有些甚至會併發症食道裂孔疝氣導致頻繁嘔吐反流等等。時間久了,因胸内負壓長期過高的狀態下,可能會走向「右心衰竭」喔!


Mystery of Nasopharyngeal Stenosis

Although recovered from an upper respiratory tract infection as a kitten, many cats would still display obvious nasal noise while breathing; furthermore, they would even show open-mouth breathing or paradoxical breathing when they are too excited. Strangely, these cats may not have any nasal discharge, and those symptoms remain more or less the same. What is going on?

Nasopharyngeal stenosis is one of the possible causes of such symptoms.

Nasopharyngeal stenosis in cats is usually an acquired condition. The pathogenic mechanism of such a condition is still unclear; however, most literatures suggest that a part of respiratory mucosa is under chronic irritation (e.g., infection, trauma) which results in subsequent repairing, adhesion, and formation of scar tissue, leading to narrowing and blockage of the nasopharyngeal passageway.

Diagnosis of nasopharyngeal stenosis is rather straightforward, either with a Nasopharyngeal Endoscope or Computed Tomography (CT) will do, yet treatment is more complicated. Treatment options include balloon dilatation, surgical removal of the stenotic region with soft palate resection, temporary stent placement, laser scar revision, etc. The recurrence rate of treatment with balloon dilatation alone is quite high (41%)(Reference 1), as well as those cats with chronic respiratory infections. However, with thorough evaluations and planning, many patients are able to achieve good results after a single surgery.

Well, what will happen if my cat receives no treatment at all?

Many cats displaying open-mouth breathing, together with abdominal breathing or sleeping disorder, often only have a nasopharyngeal opening of a size pinhole. Hiatal hernia as a common comorbidity would further result in frequent vomiting and reflux. Over time if the thoracic pressure remains abnormally high, it may progress to cardiovascular disorders e.g., right heart failure!
